The Kuchisake Onna walks up to its target and asks, "Am I beautiful?" If he answers in the affirmative, the yōkai removes its face mask to reveal its horrifically gashed, hyperdontic mouth and asks, "Even like this?". The victim will usually answer no or scream, at which point the yōkai will use the victim's shock to its advantage and slash their mouth with a scalpel to resemble its own. If the victim answers no to the first question, the Kuchisake Onna will slash their mouth anyway. Trying to play its game and answering yes to the second question won't help—the yōkai will walk away for the minute, but it will stalk the victim until he returns home, then eviscerate him in the night.
